Insurance:
Clients ask us all the time how much their insurance will cover for a visit or a procedure. The percentage of coverage depends upon the specific insurance plan. Different carriers have different fee schedules. An insurance company may have a reduced fee schedule for dental services that may be less than the actual charges, resulting in lower coverage for you. We have no control over this situation. The level of reimbursement is a direct result of the plan selected by your employer. No insurance company will pay 100% of dental costs. As a courtesy to our patients, we will process any insurance forms. Once we receive a predetermination of benefits from the insurance company, we will accept it as partial payment. You will be responsible for the remaining balance.

Forms:
Our office hours are Monday - Thursday 8:00 a.m. to 4:30 p.m., and Friday 8:00 a.m. - Noon. We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you the same day.
Please assist us at the time of your initial visit to the office by providing the following information:
- Referral slip and x-rays from your referring dentist
- List of medications you are currently taking
- Dental insurance forms or insurance cards
The following forms can be completed at our office or you may choose to print them, fill them out at your convenience, and bring them with you for your first visit.
